  • Blue Star Pest Control is an employment-at-will Company. Either you or the Company can terminate our employment relationship at any time, for any reason, or for no reason at all, with or without notice. If requested by the management at any time, I agree to submit to a search of my person or of any area that may be assigned to me, and that I hereby waive all claims and damages on account of such examination. I understand that I may be required to take a physical exam to determine consistent with business necessity, whether I am physically fit to perform the essential functions of the job for which I have applied and that the conditional offer may be revoked based upon the results of that exam. If hired, I also understand that I may be required to take a physical exam at any time, provided the exam is job-related and for a business necessity, at company expense, and I authorize any hospital or physician to release any lawful information which may be necessary to determine my ability to perform the duties of a job I am being considered for, either prior to, or during my employment with the Company in conformance with the Health Insurance Portability Accountability Act (HIPAA).
